South West Aboriginal Medical Service (SWAMS) is recruiting for a Social Emotional Wellbeing Coordination Worker.
A compassionate, culturally aware, and community-driven individual is sought to support clients on their journey to better social and emotional wellbeing.
The SEWB Care Coordination Worker is responsible for ensuring timely and accurate assessment of client needs, followed by effective linkage to appropriate internal and external social and emotional wellbeing services that support positive health and wellbeing outcomes.
It’s a full time role based in Bunbury.
