Karla Kuliny Aboriginal Corporation is seeking an Intensive Case Manager.
The Intensive Case Manager – On Track to Thrive delivers intensive, in-home family support and case management services. The role is responsible for providing culturally safe, trauma-informed practice, ensuring the consistent integration and application of the Doolann Model of Care across all aspects of service delivery.
The position leads the development, implementation, and ongoing review of Support Plans for children and their families or carers participating in the program, facilitated through regular support team meetings and targeted engagement activities.
It’s a full time role based in Boorloo/Perth. The annual salary range is $95,000 to $97,378.
